Coined in 2011 by diehard fans Del Reid, Breyon Harris, and Leslie Wille, "Bills Mafia" started as an inside joke on X (formerly Twitter). The term quickly resonated, evolving into a widely embraced identity, even garnering official recognition when the Bills filed to trademark it in 2020. As Ron Raccuia, former executive vice president of the Bills' parent company, Pegula Sports and Entertainment, explained at the time

"We felt like we needed to embrace this, because it had really turned into a community spirit." For this fan base, "Mafia" truly means family.

The genesis of the name is rooted in a unique online interaction following a 19-16 overtime loss to Pittsburgh on November 28, 2010. After Bills receiver Stevie Johnson posted a viral tweet questioning his faith after dropping a potential game-winning touchdown pass, NFL insider Adam Schefter retweeted it. This led Reid and his friends to playfully troll Schefter with old news updates labeled with the hashtag #SchefterBreakingNews.

When Schefter subsequently blocked them, Reid rallied fans to follow those blocked in the summer of 2011, eventually coining #BillsMafia. The momentum grew swiftly, with star players like Stevie Johnson, Nick Barnett, and Fred Jackson adopting the phrase, solidifying its place in Bills lore.

While the image of dedicated fans jumping through folding tables at pregame events is synonymous with Bills Mafia, it's their profound generosity that truly sets them apart. Though not a formal charity, the fan base consistently mobilizes incredible support for various causes, embodying the belief that "Mafia" means family, extending far beyond the gridiron.

Their philanthropic efforts are well-documented and often spontaneous. In 2017, after the Cincinnati Bengals won a late-season game that helped the Bills sneak into the playoffs for the first time in 17 years, Bills fans inundated Bengals quarterback Andy Dalton's foundation for seriously ill children with over $415,000 in donations. In return, the Dalton family donated to the pediatric department of Buffalo's Roswell Park Comprehensive Cancer Center.

Similarly, when Baltimore Ravens quarterback Lamar Jackson was unable to finish a 2021 playoff game in Buffalo, Bills fans donated nearly half a million dollars in his honor to the Louisville chapter of Blessings in a Backpack. This cause resonated deeply, reminding them of beloved Bills superfan Ezra "Pancho Billa" Castro, who died from cancer in 2019, and whose dying wish was for people to fill backpacks for children in need – a tradition carried on annually as "Pancho's Packs."

Perhaps the most poignant example is the more than $1.4 million raised for John R. Oishei Children’s Hospital in Buffalo in 2019 after quarterback Josh Allen lost his grandmother. These donations led to the establishment of the Patricia Allen Pediatric Recovery Wing on the hospital’s 10th floor.

As Del Reid eloquently summarized in 2021, "Your fandom can change the life of someone else for the better. That’s what’s so awesome about Bills Mafia, is that we've all kind of embraced this perspective." Kristen Kimmick, founder of Bills Mafia Babes, added

"Buffalo Bills fans and charity kind of go together at this point, like peanut butter and jelly... It's literally like the cool thing to do in Buffalo is to be kind and charitable."
